Criteria

• Appearance (colour): 0 blue/pale, 1 body pink extremities blue, 2 completely pink
• Pulse (heart rate): 0 absent, 1 below 100 bpm, 2 at or above 100 bpm
• Grimace (reflex irritability): 0 no response, 1 grimace or weak cry, 2 cry/cough/sneeze/pull away
• Activity (muscle tone): 0 limp, 1 some flexion, 2 active motion
• Respiration: 0 absent, 1 weak or irregular, 2 strong cry

Interpretation

7-10 reassuring · 4-6 moderately abnormal, may need stimulation or oxygen · 0-3 low, needs immediate resuscitation.
